Quezon City
There are two ways to register for COVID-19 vaccination in Quezon City, according to a Facebook post of the city government.
The first is through online self-booking, wherein you will be required to sign up as a patient at the EZConsult website.
The EZConsult App can also be downloaded in the App Store or Google Play.
According to an advisory from the QC government, “your account will be under review until you are eligible based on the DOH priority list.”
QCitizen ID will not be required in registering through EZConsult, but your email address and mobile number will be needed for registration.
The other way is through assisted booking, which will require your QCitizen ID.
Under this procedure, your barangay will schedule your COVID-19 vaccination based on the priority group you belong to. A representative of the barangay will contact you and provide your vaccination schedule.
QCitizen ID registration sites at the Quezon City Hall, Barangay Halls, and QCitizen ID kiosks are currently closed due to the ongoing enhanced community quarantine, but online registration can be done through https://qceservices.quezoncity.gov.ph/.
