Polling stations opened as scheduled at 6:30 a.m. (1030 GMT) in the rural, mountainous state that carries the prize of 28 delegates distributed proportionally based on the results of the vote.
An opinion poll released before the primary gave Clinton a massive lead of 36 percentage points over Obama, who has made few appearances in the state.
The former first lady, a New York senator, has campaigned extensively up and down West Virginia and argued the state is crucial for Democrats' hopes to take back the White House in the November election.
Obama holds a decisive lead in the race for the Democratic presidential nomination after contests in states across the country. But Clinton has vowed to stay in the race despite long odds.
